At FCD Health, including the Palmerston GP Super Clinic and the Palmerston Medicare Urgent Care Clinic, we are committed to ensuring that the referral and communication process is smooth and efficient for both patients and referrers.
This page has all the information you need to understand what happens with a referral, how to submit one, and what to include for a Medicare-eligible service.

When your doctor refers you to a specialist or allied health professional, we ensure the process is straightforward. Referrals are securely sent using HealthLink, a digital system designed for fast and safe communication. If you want a paper referral to take with you, just ask your doctor to print you one before you leave your appointment.
You will be contacted promptly to arrange your appointment or to receive advice on the next steps. If you are eligible for Medicare rebates, your doctor or our team will inform you.
For Darwin Nutrition and Dietetics Clinic, a referral is only required if the service qualifies for a Medicare rebate.

We encourage health providers to use HealthLink for a secure and efficient referral process. If HealthLink is not available, referrals can be emailed to us. Please ensure that referrals are complete and contain all necessary information to meet Medicare requirements.
How to Send Referrals
Please send referrals through the HealthLink network via HealthLink (Preferred). For support, contact HealthLink at 1300 145 465.
If you need to send via email, referrals can also be sent to referrals@fcdhealth.org.au.
From 28 January 2025, faxed referrals will no longer be accepted.
What to Include in a Referral
To ensure the referral is Medicare eligible, include:
- Patient information: Full name, date of birth, contact details, and Medicare number.
- Clinical details: Reason for referral, relevant medical history, and current medications.
- Request details: Specify the service or specialist required.
- Referral period: Clearly state if the referral is for 12 months or indefinite.
Confirmation of Receipt
It is essential to check that referrals have been received. If using HealthLink, acknowledgements can be monitored within the system.
